The Scotland international left-back sustained the injury in the first half of the game at the iPro Stadium.
He underwent a scan on Wednesday and is due to visit the same surgeon that operated on Will Hughes, who suffered a similar injury earlier in the season, over the coming days.
Head Coach Paul Clement said: “It’s disappointing for Craig to suffer an ACL injury and he’s set to miss the rest of the season.
“It was a freak incident and he’ll visit the surgeon in the coming days to be assessed.”
Forsyth has made 127 appearances for Derby since joining from Watford in 2013, initially on loan.
